29 CFR  1926.102(a)(1): The employer had not ensured that each affected employee had used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 30, 2023, L&A Roofing LLC did not ensure that each employee operating a pneumatic nail gun used appropriate eye protection when exposed to eye hazards from flying particles and debris in a worksite at 2050 Peine Forest Drive in Wentzville, Missouri.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels had not been protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 30, 2023, L&A Roofing LLC did not ensure that each employee roofing a single-story residential building at heights over ten feet above the ground was protected by guardrail systems or personal fall arrest systems in a worksite at 2050 Peine Forest Drive in Wentzville, Missouri.

29 CFR  1926.59: The employer had not developed a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) as incorporated by reference:  29 CFR 1910.1200(e)(1): Employers had not developed, implemented, and maintained at each workplace, a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets (SDSs), and employee information and training would be met.  On or about October 30, 2023, L&A Roofing LLC did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraph (f) for labels and other forms of warnings, (g) for SDSs, and (h) for employee information and training would be met. The chemical gasoline was used by employees to power air compressors for pneumatic nail guns in a worksite at 2050 Peine Forest Drive in Wentzville, Missouri.
