OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to amputation hazards while operating a circular saw:  a. On or about 10/24/2023, at 3415 North 55th Street, Tampa, Florida, 33619, an employee was using his hands to hold a piece of lumber which was improperly supported, while another employee was operating a hand-held circular saw to cut the lumber into shims, wherein the employee's hands were on the piece of lumber and not on the saw; therefore, having exposed the employee who was holding the piece of lumber to amputation hazards.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:  a. On or about 10/24/2023, at 3415 North 55th Street, Tampa, Florida, 33619, an employee was using his hands to hold a piece of lumber which was improperly supported, while another employee was operating a hand-held circular saw to cut the lumber into shims, wherein the employer had not instructed the employees in the recognition and avoidance of this unsafe condition; therefore, having exposed the employee who was holding the piece of lumber to amputation hazards.

On or about 10/02/2023, at 7075 W. Riverbend Road, Dunnellon, Florida, 33343, an employee was operating a table saw/circular hand-fed ripsaw to cut pieces of wood, during which time the saw was not equipped with an automatically adjusting hood/safety guard; therefore, exposing the employee to an amputation hazard.
