On or about October 30, 2023, the employer allowed an employee to operate a powered industrial fork truck prior to receiving training, exposing the employee to struck-by hazards. On or about October 30, 2023, the employer used a Rough Terrain Fork Truck to support a scaffold platform (wood pallet) that was not designed by the manufacturer for use, exposing employees to a scaffold collapse. On or about October 30, 2023, employees were not protected from a fall while working from the forks of an elevated Rough Terrain Fork Truck, exposing employees to a fall of approximately 15-20 feet.
