The employer did not ensure that eye and face protection was worn by affected employees to protect against flying particles, on or about 10/27/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 114 North 5th St Saddlebrook, NJ : Employees engaged in roofing activities on a 8 on 12 pitch roof were exposed to fall hazards of approximately 25ft to the ground below in which personal fall arrest harnesses were not being utilized and not secured to an anchor point to prevent fall hazards, observed on or about 10/27/2023.
