29 CFR 1904.29(b)(1): The employer failed to adequately describe the location and/or provide a descriptive description of a recordable injury and illness on the OSHA Form 300 or equivalent.   Cold Front Distribution - 2655 East Richmond Street, Shawano, Wisconsin.  On or about October 31, 2023, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2023.  (a) On or about October 16, 2023, while a Warehouse Picker was working, an employee sustained a sprain (Case log Number WC413F60498), which resulted in 3 days job transfer or restriction. The Employer's OSHA 300 Form or equivalent for the calendar year 2023 did not contain a complete description of the injury or illness, parts of the body affected, and object/substance that directly injured or made the person ill.

29 CFR  1910.37(b)(2):Each exit must be clearly visible and marked by a sign reading "Exit."  a) On or about October 31, 2023, at the establishment located at 2655 East Richmond Street, Shawano, Wisconsin; in Freezer 4, the employer did not ensure that an "Exit" sign was present above both exits, exposing employees to hazards associated with not being able to exit the building in the event of an emergency.

29 CFR  1910.37(b)(4): If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times.  a) On or about October 31, 2023, at the establishment located at 2655 East Richmond Street, Shawano, Wisconsin; in Freezer 2 and Freezer 3, employees were performing tasks in aisles without the aid of emergency exit access signs that show the direction to emergency exits, exposing employees to hazards associated with not being able to exit the building in the event of an emergency.
