29 CFR 1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 90 Church Street, Richfield, PA, Roof, - Employees performed commercial roofing activities on the Richfield United Methodist Church, approximately 20 feet above grade with an 6:12 roof pitch, without utilizing any means of fall protection, on or about October 31, 2023. (b) 90 Church Street, Richfield, PA, Roof, - Employees performed commercial roofing activities on the Richfield United Methodist Church, approximately 12 feet above grade, without utilizing any means of fall protection, on or about October 31, 2023. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 90 Church Street, Richfield, PA, Roof, - Employees performing commercial roofing operations on the Richfield United Methodist Church back roof, a portable extension ladder was used to access the roof. The ladder did not extend 3 feet above the upper landing exposing employees to a fall hazard of 12 feet, on or about October 31, 2023. No abatement certification or documentation required.
