The employee(s) worked approximately 4 ft from the roof edge, exposed to the hazard of falling more than 35ft to ground below.

The employee(s) worked approximately 4 ft from the roof edge, exposed to the hazard of falling more than 35ft to ground below, and had not been provided training on fall hazard prevention, including how and when to use personal fall arrest equipment while installing solar panels near an unprotected roof edge.
