Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a) Located at 3990 East Broad Street, Columbus Ohio, 43213 at building 20: On or about October 25, 2023, the employer failed to report an in-patient hospitalization of an employees within 24 hours.  An employee was hospitalized for an injury experienced as a result of a work-related incident at approximately 3pm on October 25, 2023 for surgery. The employee was released from the hospital at approximately 10 am on October 26, 2023 and the employer did not report the in-patient hospitalization until 2:55pm on October 30, 2023.
