29 CFR 1926.416(e)(1):Worn or frayed electric cords or cables shall not be used. a) On or about October 31, 2023, the employer did not ensure that frayed electrical cords were not utilized by employees installing framing material on the structure. As a result, employees were exposed to the hazard of electrical shock.

1926.503(a)(1) The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a.) On or about October 31, 2023, the employer did not ensure employees were protected from fall hazards while they installed roofing materials from a 27' 6-12 pitched roof, exposing employees to falls.
