Toledo, Ohio 43612: On or about November 1, 2023, the employer did not ensure forklifts were examined/inspected daily to detect any condition in which would adversely affect the safety operation of the vehicle before being placed in service. Employees operating the units in this condition were exposed to potential injuries associated with equipment malfunctions such as but not limited  to a non-working horn.

Toledo, Ohio 43612: On or about November 1, 2023, the employer did not ensure an attached black work platform was securely attached to the forks or carriage of a Hyster forklift, Model # S50XM while employees were picking merchandise from shelves at heights up to 23 feet above the ground below.  b.  Young's Inc. worksite located 6120 N. Detroit Rd. Toledo, Ohio 43612: On or about November 1, 2023, the employer did not ensure a yellow work platform was securely attached to the forks or carriage of a Nissan 65 propane forklift while employees were picking merchandise from shelves at heights up to 23 feet above the ground below.
