29 CFR 1910.146(d)(4)(i):  Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide or ensure that employees used the testing and monitoring equipment properly:  (a) Bay 4:  On October 23, 2023, an entrant and entry supervisor were provided with a 2-foot hose attached to a Gas Alert Max XTII for measuring the atmosphere inside the air space of a 5,000-gallon tanker trailer that an employee was entering for an inspection.  The 2-foot hose was only sufficient to measure the top airspace, with the provided tube length, exposing the employee (entrant) to hazardous airspaces once inside the tank.
