29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type and not designed to be connected to one another were engaged to each other. a) At the work site located at Rio Limpion Ln, Nokomis, Florida: On or about November 1, 2023, employees were exposed to a fall hazard of 51-ft to the lower level, in that, the fall protection system provided was not installed as intended by the manufacturer as the self retracting life-line used was attached to a free-fall lanyard and not to the anchor, while engaged in siding work on a 8:12 roof.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: a) At the work site located at Rio Limpion Ln, Nokomis, Florida: On or about November 1, 2023, employees were exposed to a fall hazard, in that, employees engaged in roofing activities were using temporary anchor points that were not properly installed according to the manufacturers installation instructions.
