On or about September 13, 2024, the employer failed to submit documentation and certification of corrective actions for serious citation 1, item 1, and repeat citation 2, item 1 issued on Aoril 26, 2024, with an abatement due date of June 13, 2024.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a. On the low slope metal roof of the single-story commercial building, at 1530 State Avenue, Holly Hill, FL: On or about October 31, 2023, the employer exposed an employee to an 18-feet fall hazard, in that personal fall arrest systems, covers, or guardrail systems were not provided to protect the employee from the hazard of falling through unguarded skylights while applying silicone coating material.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a. On the low slope metal roof of the single-story commercial building, at 1530 State Avenue, Holly Hill, FL: On or about October 31, 2023, the employer exposed an employee an 18-feet fall hazard, in that a fall protection system was not in use while performing application of the silicone coating of the roof leaks.  Amigo Roofing and Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1655846, citation number 01, item number 001, and was affirmed as a final order on 5/17/2023, with respect to a workplace located at 2201 N. Dirksen Pkwy Circle K Car Wash, Springfield, IL 62702.
