29 CFR 1910.23(c)(7)(i): The employer did not ensure that ladders placed in locations such as passageways, doorways, or driveways where they can be displaced by other activities or traffic are secured to prevent accidental displacement On or about October 30, 2023, at a street parking area, an extension ladder was setup on the side rack of a truck and extended over the sidewalk. The extension ladder was not secured to prevent displacement when an employee worked from the ladder to caulk windows. The employee was exposed to fall hazards.

29 CFR 1910.23(c)(13): The employer did not ensure that ladders are not placed on boxes, barrels, or other unstable bases to obtain additional height. On or about October 30, 2023, and at times prior to, at a street parking area, the feet of an extension ladder were placed on a side rack of a truck to gain additional height when an employee worked from the ladder to caulk windows. The employee was exposed to fall hazards.
