At a temporary residential worksite, located at 4561 Allure Lane - Building 15, Jacksonville, FL 32246: On or about November 2, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection were not utilized by the employees, while using nail guns to install sheathing and a saw to cut sheets of plywood. b. At a temporary residential worksite, located at Allure Lane - Building 10, Jacksonville, FL 32246: On or about November 2, 2023, the employer exposed an employee to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee while he was using a nail gun to install roofing shingles.

At Building 10, On a residential worksite, located at 4561 Allure Lane, Jacksonville, FL 32246: On or about November 2, 2023, the employer exposed employees to fall hazards, in that, fall protection were not utilized by the employees walking the sil plates and working near edge of buildings. The employees were being exposed to fall hazards up to 18 feet 7-inches. b. At Building 15, On a residential worksite, located at Allure Lane, Jacksonville, FL 32246: On or about November 2, 2023, the employer exposed an employee to fall hazards, in that, fall protection was not utilized by the employees installing roof shingles. The employee was being exposed to fall hazards up to 30 feet 4-inches.
