OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck-by hazards: (a) On or about October 28th, 2023, at or near Well Pad SC-4WX-153-98-3130H-7 Watford City, North Dakota, an employee was exposed to struck-by hazards while fixing a cover on the drilling rig while the top drive was moving causing the employee to get pulled onto the top drive. Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to: 1. Follow the American Petroleum Institute (API) Recommended Practice 54, section 6.5, and/or the American National Standards Institute (ANSI) Z 244.1 Control of Hazardous Energy Lockout/Tagout and Alternative Methods, Section 4.2 "Risk Assessment, and Section 5 "Hazardous Energy Control Program."

(a) On October 28th, 2023, at Well Pad SC-4WX-153-98-3130H-7 Watford City, North Dakota, an employee was hospitalized after being pulled onto the top drive of an oil drilling rig but was not reported to OSHA within 24 hours.
