Alex Argueta Mendez located at 868 East College Ave Westerville, OH 43081: On or about October 24, 2023, the employer did not ensure that portable ladders used to gain access to the roof extended at least three feet beyond the upper landing that they were using for access. One extension ladder was used to gain access to roof of the garage was positioned approximately 18 inches beyond the upper landing. The following condition exposed employees to falls over 9 feet.
