29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about October 30, 2023, at the above addressed jobsite, employees conducting commercial roofing operations from a ladder jack scaffold, sixteen feet (16) above the ground, were exposed to fall hazards due to lack of a guardrail system.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)   a. On or about October 30, 2023, at the above addressed jobsite, employees performing commercial roofing operations, nineteen (19) feet two (2) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
