29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service. a. On or about October 31, 2023, employees were required to use fall protection equipment during tank cleaning operations which was not inspected prior to use, thereby exposing employees to fall hazards. Equipment in used (Honeywell H500/EN362 Carabiner) has the following defects: Corrosion on the ring attached to the Candor strap.
