On November 1, 2023, employees engaged in decking activities without fall protection were exposed to unprotected fall hazards of approximately 9 feet to the ground below.

On November 1, 2023, the employer did not provide an extension ladder where employees could access and egress the residential building roof level, exposing them to potential fall hazards.
