The employer did not ensure eye protection was utilized by employees operating pneumatic nail guns. Employees were exposed to struck-by hazards.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On or about November 2, 2023, employees of Poncho Ruiz, dba: Poncho Ruiz were performing roofing work at 125 Brookshire Creek Drive in Wentzville, Missouri. The employer did not ensure that fall protection was worn while work was being performed on the roof approximately 12.5 feet above the lower level. The employees were exposed to fall hazards to the ground below.
