Employees are periodically required to set up injection molding machines.     B.) PTR Tool & Plastics, LLC, On or about 11/2/23 and times prior thereto: Employees did not lockout prior to conducting servicing operations on equipment including but not limited to, injection molding machines.  Employees are periodically required to clear jams on injection molding machines.      C.) PTR Tool & Plastics, LLC, On or about 7/31/23 and times prior thereto: Employees did not lockout prior to conducting servicing operations on equipment including but not limited to, injection molding machines.  Employees are periodically required to clear jams at the pinner.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
