29 CFR 1904.30(a): The employer did not keep a separate OSHA 300 Log for each establishment that was expected to be in operation for one year or longer: Sysco Baraboo, LLC. - 6106 Chuck Ln, Eau Claire, WI. Separate OSHA 300 Logs were not maintained for this establishment. (a) On or about October 17, 2023, an employee felt pain while bending over to lift a five-gallon container of product resulting in five days of job transfer or restriction, log case no. 335280. The employer included the case on a single log used for the company for calendar year 2023.
