29 CFR  1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   a) Yard: On or about October 31, 2023 and at times prior, employees were exposed to struck by hazards while pulling skids with the modified fork of the M36 Forklift.  The employer failed to obtain manufacturers prior written approval prior to modifying the forks.  b) Yard: On or about October 31, 2023 and at times prior, employees were exposed to struck by hazards while pulling skids with the modified fork of the M43 Forklift.  The employer failed to obtain manufacturers prior written approval prior to modifying the forks.
