29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Worksite (Chapel Road, Mattapoisett, MA): When employees were performing roofing activities in excess of six feet above ground level, the employer did not ensure that all of the workers utilized fall protection when on the roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   Worksite (Chapel Road, Mattapoisett, MA):  In the following instances, employees conducting roofing activities, and exposed to falls in excess of ten feet above ground level, were not adequately trained to recognize the procedures to be followed to minimize the hazards:  1) A worker conducting roofing shingling activities at the edge of the roof, attached his lanyard to the side ring of his harness instead of the "D" ring on the back of his harness.  If he fell, he could incur life-threatening injuries to his spine and internal organs. 2) Before coming down from the roof, one of the workers tied off his co-worker inappropriately to prevent a fall.  The  Co-worker's lanyard had no hook, so the worker tied the end of the rope lanyard into a  knot on the co-worker's "D" ring  before she began her descent.  This is not an approved method of preventing a fall to the ground. 3) Employees were not adequately trained to set up ladder access to the roof.  Both ladders used by the workers did not have railings at least 3 feet above the roof level, which allows for safer access and egress.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Worksite (Chapel Road, Mattapoisett, MA): When employees were conducting roofing activities in excess of ten feet above ground level, the employer did not ensure that the rails of the ladders used to access the roof extended at least 3 feet above the roof's landing.
