29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about November 1, 2023, in the Hog Fuel Boiler area, employees engaged in maintenance activities such as, but not limited to, installing tubing and wiring on the boiler control valve system.  Hazardous energy control procedures were not utilized.

29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):   On or about November 1, 2023, near the Hog Fuel Boiler area, employees performed servicing and maintenance activities such as installing, setting up, adjusting, inspecting, maintaining, repairing, cleaning and servicing equipment related to the Hog Fuel Boiler. Procedures developed and documented did not include specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  a) Employees engaged in maintenance activities such as, but not limited to, installing tubing and wiring on the boiler control valve system.   b) Employees engaged in maintenance activities such as, but not limited to, replacing a valve on the boiler control valve system.

29 CFR 1910.147(f)(3)(ii)(C): More than one crew, craft, department, etc. was involved and the employer did not assign overall job-associated lockout or tagout control responsibility to an authorized employee designated to coordinate affected work forces and ensure continuity of protection:   On or about November 1, 2023, an authorized employee was not assigned to oversee the coordination between employees including, but not limited to maintenance planners, operators, and technicians when engaged in the maintenance activities related to the Hog Fuel Boiler control valve system.

Procedures were not utilized to ensure continuity of lockout during personnel changes.

29 CFR 1910.261(b)(1): The employer did not ensure that the main power disconnect switch or valve, or both, controlling its source of power or flow of material, was locked out or blocked off with padlock, blank flange, or similar device before conducting any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that required entrance into or close contact with the machinery or equipment:   On or about November 1, 2023, at the Hog Fuel Boiler, employees accessed the control valve to perform maintenance tasks, such as, but not limited to hooking up an airline and electrical wires, where it was not shut down and locked out.
