29 CFR 1910.28(a)(1): The employer did not provide protection for each employee exposed to fall and falling object hazards: a) Outside Dock/Area - On or about October 30, 2023, employees were exposed to a falls between 4 feet and 4 feet 6 inches when wrapping chains around containers on the bed of flatbed trucks when preparing for the unloading of the containers. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHAs public web site at www.osha.gov: a) Yunexpress USA Inc. - On or about October 30, 2023, the employer did not report an in-patient hospitalization that resulted from a work-related incident, to OSHA within twenty-four (24) hours. The injury was reported to OSHA on November 1, 2023. No abatement certification or documentation required for this item.
