This condition exposed employee to caught-between hazards.

Electrically or manually operated quick power disconnecting devices, interlocked with braking action, were not provided on all operating sides of the machine within easy reach of all employees. On or about November 1, 2023, employer did not ensure to install an emergency stop button on conveyor belt. This condition exposed employee to caught between hazards.
