29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.  On or about January 31, 2024, at the infeed side of Coater 3, the coater operator was not protected from falls by a stair rail system on the back-side stairs to the operators platform. These stairs have three treads and four risers and is open on both sides, and was not equipped with a stair rail system on either side of the staircase. Table D-2 requires such stairs be equipped with stair rail system on both open sides.

29 CFR  1910.1200(f)(6)(ii):  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  On or about January 31, 2024, at the part-cleaning station in the barrel room, employees cleaning parts in 2-butanone (methyl ethyl ketone) utilized a stationary cleaning tank that was not labeled with the information necessary. The container was filled with 2-butanone, however the label affixed was illegible, and did not describe the contents of the workplace container nor the physical and health hazards of the methyl ethyl ketone, which include flammability as well as headache, dizziness, vomiting, and irritation of the eyes, skin, and nose.
