29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   Church Roof - Five employees, engaged in roofing activities approximately 40 feet above the ground, were not protected from falling.  LMC Home Remodeling, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard1926.501(b)(13), which was contained in OSHA inspection number 1585043, citation number 1, item number 1, and was affirmed as a final order on 08/01/2022, with respect to a workplace located at 1050 Forest Street, North Andover, MA.  LMC Home Remodeling, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1363076, citation number 1, item number 2, and was affirmed as a final order on 12/19/2018, with respect to a workplace located at 69 South Street, Concord, NH.  LMC Home Remodeling, Inc, DBA:  JL Castro Construction, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1644246, citation number 1, item number 1, and was affirmed as a final order on 3/30/2023, with respect to a workplace located at 52 Main Street, Lincoln, ME.
