On top of a structure located at 3031 Spiro Circle, Jacksonville, FL: On or about November 7, 2023, the employer exposed employees to struck-by hazards, in that an employee was not wearing safety glasses while using a pneumatic nail gun.

On top of a structure located at 3031 Spiro Circle, Jacksonville, FL: On or about November 7, 2023, the employer exposed employees to a 10-foot 4-inch fall hazard, in that fall protection was not utilized by employees performing roofing operations.
