29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. On or about November 7, 2023, at location 1101 Moasis Drive Little Chute, Wisconsin: employees operating a Crown standup forklift (Model: RC5545-40, Serial: 1A577325) with damage including, but not limited to, chunks of tire missing, severe splitting of tire sidewalls and tires pulling away from the rims.
