(a) On or about November 7, 2023, employees were using a Werner multi-position ladder on the ridge of a 6:12 pitch residential roof to support a ladder jack. The ladder was configured as a step ladder with each of the four ladder's legs resting on four independent four-inch-thick foam pads positioned on either side of the sloped ridge of the roof without being equipped with devices to prevent slipping. This exposed employees to an unstable work surface and a fall hazard.
