29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about November  7, 2023, the Werner Extension  ladder erected on the West side of the structure, at the garage that was being used for access to the 6:12 pitched roof, did not extend at least three (3) feet above the eave, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately nine (9) feet.  (b) On or about November  7, 2023, the Werner Extension  ladder erected on the West side of the structure, above the garage that was being used for access to the 2nd floor 6:12 pitched roof, did not extend at least three (3) feet above the ridge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately nine (9) feet followed by a second fall hazard of an additional nine feet (9) to the ground level.

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) On or about November 7, 2023,  the employer did not ensure that the employee assisting in roofing operations (ground worker) was protected from potential falling objects from above by the use of a protective helmet. This exposed the employee to a struck-by hazard from a height of nine (9) feet by material remnants and tools above the working employee.  Nass Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1536800, citation number 1, item number 1, and was affirmed as a final order on 06-13-2022, with respect to a workplace located at 947 Leverenz Road, Naperville, IL., 60565.

29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about November 7, 2023, employees operating pneumatic nail guns on the roof of the residential structure were not wearing safety glasses,  exposing employees to a struck by hazard to the eyes.  Nass Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1536800, citation number 1, item number 2, and was affirmed as a final order on 06-13-2022, with respect to a workplace located at 947 Leverenz Road, Naperville, IL., 60565.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) On or about November 7, 2023, employees conducting roofing work on an 6:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately nine (9) feet, on the West  side of the roof, and 19 feet, 4 inches on the South side of the roof.  Nass Roofing, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1536800, citation number 1, item number 3, and was affirmed as a final order on 06-13-2022, with respect to a workplace located at 947 Leverenz Road, Naperville, IL., 60565.
