29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about November  7, 2023, the Werner Extension  ladder erected on the West side of the structure, at the garage that was being used for access to the 6:12 pitched roof, did not extend at least three (3) feet above the eave, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately nine (9) feet.  (b) On or about November  7, 2023, the Werner Extension  ladder erected on the West side of the structure, above the garage that was being used for access to the 2nd floor 6:12 pitched roof, did not extend at least three (3) feet above the ridge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately nine (9) feet followed by a second fall hazard of an additional nine feet (9) to the ground level.
