(a) At the worksite: On or about 11/7/2023, the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person, where there were fall hazards identified as well as a telehandler with inadequately connected tine extensions. The John Flatley Company was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1577378, citation number 1, item number 1 and was affirmed as a final order on 04-12-2022, with respect to the workplace located at 1259 Wampanoag Trail, Riverside, RI 02915.
