29 CFR 1926.451(b)(5)(ii):The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: (a) A scaffold stage greater than 10 feet in length, used as a work platform of a ladder jack scaffold system to sheath a roof, extended more than 18 inches over the bracket supports. The cantilevered end(s) were not properly supported and were not equipped with guardrails.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: (a) Employees utilized a scaffold stage as part of a ladder jack scaffold system work platform to sheath a roof and it extended more than 18 inches over the bracket supports, the cantilevered end(s) were not properly supported and were not equipped with guardrails, and no fall protection was utilized. A competent person was not involved.
