29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:  Site: 28-08 38th Ave, Long Island City, NY 11101. On or about November 07, 2023, employees engage in pluming work on the 4th floor were exposed to a floor hole opening that was not secured. The floor hole measured approximately 3-feet long and 2-feet wide.  Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
