29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program that provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. On or about 8 November 2023, a Gateway General Contractors' subcontractor was installing roofing materials on a residence located at 220 Victoria Park Avenue in Wentzville, Missouri. Gateway General Contractors did not conduct an adequate inspection of the job site, materials, and equipment. Six employees were observed working on a roof without utilizing fall protection. The employer did not utilize guardrail systems, a safety net system, or a personal fall arrest system.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about 8 November 2023, a Gateway General Contractors' subcontractor was exposed to fall hazards while working on the roof of a residential building located at 220 Victoria Park Avenue in Wentzville, Missouri. The employees were removing roofing materials without fall protection.
