29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. a) For ballast tank B-1-P-Aft in the barge "The Mississippi" at 1800 Grant St., Tampa, FL 33605, on or about 10/31/2023, the employer exposed employees to trip/fall hazards, in that, an access opening was not safely guarded to prevent employees from stepping/falling into it while walking/working in the area. The access opening was approximately 14 feet above the ballast tank level below it.

The employer notified OSHA on November 3, 2023 of the in-patient hospitalization.
