29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   a) Jobsite: 3114 Grays Ferry Ave. Philadelphia, PA 19146 - On or about November 7, 2023, an employee conducting roofing activities from the roof of a 5 story commercial building, approximately 63 feet above the ground level, was exposed to fall hazards without the use of a fall protection system.

29 CFR  1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function.  a) Jobsite: 3114 Grays Ferry Ave. Philadelphia, PA 19146 - On or about November 7, 2023, the employer, serving as the designated safety monitor working from the roof of a 5 story commercial building, carried out other responsibilities which took his attention from the monitoring function exposing his employees working along the open and unprotected edges of the roof of the building to approximately 63 foot fall hazards.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a) Jobsite: 3114 Grays Ferry Ave. Philadelphia, PA 19146 - On or about November 7, 2023, the employer did not provide effective training for each employee installing flashing on the roof of a commercial building and exposed to fall hazards of approximately 37 feet, in the recognition and avoidance of fall hazards.
