29 CFR  1910.28(b)(1)(i)(A):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a) Powder Plant Mezzanine Area; On or about November 7, 2023, employees were exposed to fall hazards over 4 feet, in that the walking-working surface did not contain a standard guardrail system, resulting in two unprotected sides while employees transfer storage material.  b) Powder Plant Howell Blender; On or about November 7, 2023, employees were exposed to fall hazards over 4 feet, in that the catwalk's walking-working surface did not contain a standard guardrail system, resulting in an unprotected edge while employees mix raw powdered materials.  c) Powder Plant Mike's Blender; On or about November 7, 2023, employees were exposed to fall hazards over 4 feet, in that the catwalk's walking-working surface did not contain a standard guardrail system, resulting in an unprotected edge while employees mix raw powdered materials.  d) Powder Plant Stainless Blender; On or about November 7, 2023, employees were exposed to fall hazards over 4 feet, in that the catwalk's walking-working surface did not contain a standard guardrail system, resulting in an unprotected edge while employees mix raw powdered materials.  e) Powder Plant Big Blender; On or about November 7, 2023, employees were exposed to fall hazards over 4 feet, in that the catwalk's walking-working surface did not contain a standard guardrail system, resulting in an unprotected edge while employees mix raw powdered materials.

29 CFR  1910.29(b)(1):29 CFR 1910.29(b)(1): The employer did not ensure that guardrail systems met the requirement that the top edge height of top rails, or equivalent guardrail system members, that are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section.  a) Powder Plant Howell Blender; On or about November 7, 2023, employees were exposed to entrapment and fall hazards, in that the blender's lid edge came to a height of approximately 33 inches, resulting in an unprotected edge while employees open and close the lid for cleaning purposes.

INC. FG 30G; Serial Number: 108238)  was not taken out of service or in safe and operative condition, as the front right tire has a loss of mass.  b) Powder Plant Area; On or about November 7, 2023, and at times prior, the employer exposed employees to struck by hazards, in that a sit-down forklift (Model: TAILIFT U.S.A. INC. FG 30G; Serial Number: 108238)  was not taken out of service or in safe and operative condition, as the leather seat cover was worn and lifting.
