On or about November 8, 2023, at the above addressed jobsite, an employee working below residential roofing operations was not wearing a hard hat, thereby exposing the employee to struck by hazards.

On or about November 8, 2023, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.

On or about November 8, 2023, at the above addressed jobsite, an employee performing residential roofing operations, eleven (11) feet eleven (11) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about November 8, 2023, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet five (5) inches above a lower roof, were not protected from falls, thereby exposing the employees to fall hazards.

On or about November 8, 2023, at the above addressed jobsite, the ladder used to gain access to the upper roof was not extended at least three (3) feet above the upper landing surface, thereby exposing the employees to fall hazards.
