29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At the residential construction located at 3101 NW Berkebile Lane in Lee's Summit, Missouri: Employees were exposed to electric shock, and electrocution while using a frayed extension cord to plug the pneumatic nail guns and the circular saws that had a damaged strain relief. The extension cord had wires exposed at the end connector and was missing the insulation and guarding that protects the wires.
