29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service. a. On or about Nov. 8, 2023, employees were required to use fall protection equipment during tank cleaning operations which was not inspected prior to use, thereby exposing employees to fall hazards. Equipment used (Guardian Body Harness) has the following defects: Corrosion on the dorsal D-ring, leg strap eyelets and adjustable torso buckles.
