29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Trench - On or about November 9, 2023, employees working in a trench were exposed to a fall hazard of approximately seven and a half (7.5) feet in that the portable ladder they were provided for access and egress was installed such that the side rails only extended half (0.5) a foot above the edge of the trench. Certification Not Required.
