29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c) On or about November 9, 2023, employees engaged in exterior residential plumbing activities located at 8347 Western Ave, Omaha, NE 68114 were exposed to cave-in hazards in that the unprotected employee was working in a trench that was in excess of 8-feet while conducting plumbing operations. Plumbrite LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1591977, Citation number 1, Item number 1 and was affirmed as a final order on June 17th, 2022, with respect to a workplace located at 11651 South 123rd Ave, Papillion, Nebraska 68046.

29 CFR 1910.1200(g)(1):Chemical manufacturers and importers shall obtain or develop a safety data sheet for each hazardous chemical they produce or import. Employers shall have a safety data sheet in the workplace for each hazardous chemical which they use. On or about November 9, 2023, employees engaged in exterior residential plumbing activities located at 8347 Western Ave, Omaha, NE 68114 and were exposed to chemical hazards in that the the employer failed to provide and make readily available Safety Data Sheet for the PVC glue the employees were using on the jobsite.
