29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who are exposed to hazardous or potentially hazardous conditions, equipment or operations which are out of the ordinary, unexpected or not readily apparent:    a.) Buck's Pizza, 53 Industrial Drive, DuBois, PA 15801, kitchen - On or about, 11/9/23, and at times prior to, employees were operating the pasta machine while it had flames shooting 6+ inches up the back of the Frymaster pasta machine housing.  The Frymaster pasta machine was not tagged or taken out of service until a qualified, commercial kitchen equipment repair technician could review the condition and make repairs.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Buck's Pizza, 53 Industrial Drive, DuBois, PA 15801, kitchen - On or about 11/9/23 and times prior thereto,  several sinks, dishwasher and pasta machine were being drained at the same time causing the water to overflow from the drain and onto the floor.  Safety instruction signs were not posted with instructions to not drain all of the equipment at once.
