29 CFR 1910.37(a)(4):   Exit Routes & Emergency Planning - Safeguards designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting) must be in proper working order at all times.  263 Myrtle Street (Cold Storage Warehouse & Shipping-Receiving Dock): On or about November 14, 2023, Employer did not ensure Employees were protected from accidental injury because exit lighting was not in proper working order at all times.   Cold Storage Warehouse ("Freezer") & Shipping-Receiving Dock ("Dock"): Damaged and/or partially removed "EXIT" located above an egress point between the Freezer and the Dock, was not in proper working order at all times.

29 CFR 1910.37(b)(5):   Exit Routes & Emergency Planning - Each doorway or passage along an exit access that could be mistaken for an exit must be marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g., closet).  263 Myrtle Street (Cold Storage Warehouse & Shipping-Receiving Dock): On or about November 14, 2023, Employer did not ensure Employees were protected from accidental injury because each doorway or passage along an exit access that could be mistaken for an exit or was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use.   1) Shipping-Receiving / Loading-Unloading ("Dock"): An accessible doorway along  an egress route that could be mistaken for an exit, was not identified by a sign indicating its actual use, or that the doorway identified as "EXIT", an active egress, or "Not an Exit" an inactive egress.  2) Cold Storage Warehouse ("Freezer") Side of Partition Wall: An accessible doorway along an egress route that could be mistaken for an exit, was not identified by a sign indicating its actual use, or that the doorway identified as "EXIT", an active egress, or "Not an Exit" an inactive egress.

29 CFR 1910.305(b)(2)(i):   Wiring methods, components, and equipment for general use - Cabinets, Boxes, and Fittings - All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  263 Myrtle Street (Cold Storage Warehouse & Shipping-Receiving Dock): On or about November 14, 2023, Employer did not ensure Employees were protected from accidental injury because all pull boxes, junction boxes, and fittings were not provided covers identified for the purpose. Outlet boxes did not have covers, faceplates, or fixture canopies. Junction boxes were not fully enclosed and missing fasteners on faceplates.   1) Cold Storage Warehouse ("Freezer") Side of Partition Wall: Junction box with 2x flexible cords spliced into it, was partially exposed due to missing fasteners and appeared to be intentionally left in this state to allow access to the exposed connectors.   2) Shipping-Receiving, Loading-Unloading ("Dock") Side of Partition Wall: Junction box was not fully enclosed and missing fasteners on faceplate.
