29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: a. At the Triple C warehouse located at 3525 Imeson Road, Jacksonville Florida, 32254: On November 7, 2023, and at times prior, the employer exposed employees to struck-by and crushed-by hazards, in that training was not provided to all affected employees, who the employer had directed to operate powered industrial trucks.
