At the worksite, located at 10600 Endeavour Way Largo, FL 33777; On or about 11/13/23, the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees , in that, employees were exposed to a struck-by hazard from flying chlorinated polyvinyl chloride (CPVC) pipe fragments in which the employer used CPVC piping of varying diameters to transport compressed air.

At the worksite, located at 10600 Endeavour Way Largo, FL 33777; On or about 11/13/23, the employer exposed employees to to fire and electric shock hazards, in that extension cords were used were used as a permanent method and daisy chained; connected to multiple extension cords, power strips and to an outlet across the walking and working floor surface while energizing an electric rope cutter, Demaron Sewing Machine, Zebra 4 label maker, order ipad, fan and computer in the production areas of the facility.
